COVID-19 Screener for Families and Students
For the safety of our school community, if you answer YES to any of these questions, please stay home.
Do you have or have you recently had any of the following?

  • A fever of 100.0ºF or higher, chills, or repeated shaking with chills
  • A cough that cannot be attributed to another health condition
  • Shortness of breath or difficulty breathing that cannot be attributed to another health condition
  • Muscle aches or pains (myalgias) or general fatigue that cannot be attributed to another health condition or that may have been caused by a specific activity, such as physical exercise
  • A sore throat that cannot be attributed to another health condition
  • Congestion or runny nose that cannot be attributed to another health condition
  • A headache that cannot be attributed to another health condition
  • A new loss of the sense of smell or taste
  • Nausea, vomiting, or diarrhea
  • Close contact (within 6-feet for more than 15 cumulative minutes) in the past two weeks with a person who has a confirmed case of COVID-19
  • Close contact (within 6-feet for more than 15 cumulative minutes) in the past two weeks with a person awaiting COVID-19 test results
  • Traveled out of the country in the past 14 days

You may not enter the building if you are experiencing any of these symptoms. If you are an Affton School District staff member, please call your supervisor to discuss options.
