Transcripts

Any high school may obtain transcript information regarding Affton School District students by first faxing/sending the signature release of the student to (314) 633-5983 or scanning the release and emailing to the registrar, Donna Babinec

Any post-secondary institution requesting a transcript needs to follow the directions below.

Any employer may do an education verification request with a signature release, but we cannot send a copy of the transcript unless the below is followed.

Any former Affton School District student may request a copy of their high school transcript to be sent to colleges/employers by doing one of the following:

Stop by the Affton High School Registrar’s office at 8309 Mackenzie Rd, St. Louis, MO  63123, during the hours of 7:30 a.m. to 3:00 p.m., Monday through Friday to fill out a request form, signature release, and pay the fee of $5.00

OR

Download the Release of Transcript form and mail to the attention of the Registrar at 8309 Mackenzie Rd, St. Louis MO  63123. Please provide the following information:

o Name at the time of graduation or withdrawal

o Year of graduation or intended graduation- this is how we file records so please be accurate

o Date of birth

o Complete name and address or fax number of where to send transcript

o Signature of the former student, releasing this information

o Pay $5.00 fee, per copy by cash, check or money order. Please make check or money order payable to Affton High School.

However, only an UNOFFICIAL copy can be given/sent to the actual student. OFFICIAL copies with the seal can only be sent to a business or a college because they must be in a sealed envelope.  If you wish to take it yourself, we will seal it up and give to you, but check that your destination will accept it first.

Emailed requests for transcripts or academic records by a student cannot be honored.

We cannot provide a transcript if all of the information above has not been provided.

Please note: WE DO NOT KEEP COPIES OF DIPLOMAS.  STUDENTS RECEIVE THEIR DIPLOMA WHEN THEY GRADUATE AND THEY ARE RESPONSIBLE FOR IT.
